New and Returning Student Registration Grades 1 to 12

Welcome to the Scituate Public Schools. During the month of December 2021 the link to our online registration program will be provided upon a request made to:

Scituate Public School's Kindergarten registration for the 2022-2023 school year will begin in January. Please check the Kindergarten Registration page for updates.

Scituate High School has a specific requirements for students transferring from another school. Please click here to view the Transfer Student Letter 2021-2022. Tammy Rundle, the Director of School Counseling, has her email at the bottom of the information sheet if you have questions not answered in the Transfer Information Letter.

Required Documentation for Grades 1 to 12

If you choose not to upload the documents during the online registration process you will need to send the documents to the Scituate Public Schools Central Office by mail or email. Contact the Registrar at if you need assistance providing the documents in person.

All required documents must be provided for a registration to be considered complete.

Child's current immunization record

Copy of Original Birth Certificate for grades 1 to 12

Previous School Records: Release of Records Request Form

Three (3) Proofs of Scituate Residency and Identification.

Proof 1: Evidence of Residency: Please provide One (1) of the following:

  • Recent mortgage payment and/or property tax bill
  • Fully signed and executed Lease and/or Rental Agreement (Must be executed by both parties)
  • Landlord/Owner of Property Affidavit (Click here for the Residency Statement/Affidavit) for those who cannot produce proof of ownership or Lease.
  • Fully signed and executed Purchase and Sale (P&S) Agreement (provided occupancy date occurs within 30 days of enrollment)
  • Section 8 Agreement

Proof 2: Evidence of Occupancy: Please provide One (1) of the following:

  • Gas/Oil Bill, Electric Bill, Home (not cell) Telephone Bill, Cable Bill, Water Bill (Note: Bill must be dated within the past 45 days and address and name must be stated)
  • Recent bill dated within the past 45 days showing Scituate address and name (Note: A Residency Statement/Affidavit is required with this option)
  • Occupancy Statement/Affidavit must be notarized if a bill cannot be provided prior to student's enrollment.

Proof 3: Identification of Parent/Guardian: Please provide a copy of One (1) of the following:

  • Valid MA Driver's License
  • Valid MA Photo ID Card
  • Valid Passport
  • Other Government issued Photo ID

If your student will need transportation, please complete an application and submit with payment (if necessary) as soon as possible. Space is limited, and if the bus route is at capacity, you may be placed on a waiting list. Application and other transportation information can be found on the Transportation link below:

Contacting the Registrar - location to drop off documentation

Registration is located at the Scituate Public School's Central Administration Office which is on the Gates Middle School/Scituate High School campus. You can scan and email the completed forms to For required documents not emailed or for those needing to be photocopied, please contact the Registrar to make an appointment to complete your child's registration.

Registration is complete once all forms and documents are approved by the Registrar.

Student Birth Certificate

You may provide a copy of your child's original Birth Certificate or child's Passport for Grades 1 to 12. For Kindergarten students an original must be presented, with a seal, at time of registration.

Free and Reduced Lunch

The Scituate Public Schools provides free or reduced price meals to student whose families meet the financial requirements. You can access additional information and forms at

Race and Ethnicity

The Online Registration process requests Race and Ethnicity information for your child based on the following definitions:

  • Hispanic or Latino: A person of Cuban, Mexican, Puerto Rican, South or Central American, or other Spanish culture or origin, regardless of race. The term, "Spanish origin," can be used in addition to Hispanic or Latino.
  • Or, Not Hispanic or Latino
  • Race (you may select all that apply):
  • American Indian or Alaska Native: A person having origins in any of the original peoples of North and South America (including Central America), and who maintains tribal affiliation or community attachment.
  • Asian: A person having origins in any of the original peoples of the Far East, Southeast Asia, or the Indian subcontinent including, for example, Cambodia, China, India, Japan, Korea, Malaysia, Pakistan, the Philippine Islands, Thailand, and Vietnam.
  • Black or African American: A person having origins in any of the black racial groups of Africa.
  • Native Hawaiian or Other Pacific Islander: A person having origins in any of the original peoples of Hawaii, Guam, Samoa, or other Pacific Islands.
  • White: A person having origins in any of the original peoples of Europe, the Middle East, or North Africa.

Military Family Status

As part of the Interstate Commission on Educational Opportunity for Military Children the State of Massachusetts requires school districts to collect Military Family status information of a student's parent or guardian. The term uniformed services used in the registration form means the Army, Navy, Air Force, Marine Corps, Coast Guard as well as the Commissioned Corps of the National Oceanic and Atmospheric Administration, and Public Health Services.

Student medical information must be completed and reviewed by a Scituate Public School nurse prior to a student attending their first day of school in Scituate.
