New Student Required Forms - Required forms for new/transfering students.

Transcript_Release_Form_2012 - Seniors must complete and return this form to the School Counseling Department in order to process and send transcripts to colleges.

Alumni_Transcript_Release_Form - Transcript release form for SHS alumni

Community_Service_Form - Students must complete and return to their Counselor.  50 hours of community service must be submitted by the end of first semester of senior year.

First_Quarter_Release_Request_Form - Seniors need to complete in order to send first quarter grades to colleges to which they are applying.
 
Parent_Brag_Sheet - Senior parents can provide information to their student's counselor to help support the Counselor recommendation letter.

Consent or Denial of Consent Form 2010 - Due by October 1st. All juniors and seniors must submit this form.  Refer to Program of Studies for more details.
