ATTENTION ALL HIGH SCHOOL SPRING ATHLETES!!!!
All athletes are required to turn in a current physical from their doctor, $300.00 athletic fee for the spring season, emergency medical contact information form, and permission to participate form. Please access the forms using this link: SHS Athletic Forms. The forms will also be available at the main office during school hours.
Baseline concussion testing permission forms are also be available. All athletes are encouraged to have a concussion baseline test. Tests are performed here at SHS. Parents and students who plan to participate in any athletic program at Scituate High School must also take a free on-line course at one of the following two websites:

ABSOLUTELY NO STUDENT will be allowed to participate in any sport without a current physical turned in before the season begins. No tryouts, no practices and no games. If you are unsure if your child has turned in a current physical you can contact the school nurse at 781-545-8750x380.
